Prediction of Cardiovascular Disease among Women of Reproductive Age: A Case Study of Novel Methods in Perinatal Research

Risk prediction models have been widely used in perinatal epidemiology to estimate the risk of outcomes that occur prior to pregnancy (e.g., success of infertility treatments), during pregnancy (e.g., preeclampsia) and delivery (e.g., success of a vaginal delivery after a caesarean section) and in the post-partum period (neonatal admissions and mortality). The availability of large databases has brought new opportunities for risk prediction modeling, including the availability of data on a large number of individuals followed longitudinally over long periods of time and linkage to multiple data sources. However, these data frequently include multiple pregnancies per woman which presents challenges for the development of risk prediction models. Using an example from my research on the influence of pregnancy complications on the long-term cardiovascular health of women, I will describe the methodological approaches I used to address the challenges of incorporating reproductive and perinatal risk factors in prediction models and the transportability of models across populations.
