Despite the several coordination and orientation activities under way, the patient’s journey remains at the heart of the CCAR’s (Centre de coordination des activités réseau) objectives. From the various ongoing projects, two specific achievements are reflective of this vision.
Thanks to the emergency line set up by the CCAR nearly three years ago, the CHU Sainte-Justine Traumatology team was able to deploy a trauma activation code. This code provides the doctor referring an unstable trauma case with quick access to the entire multidisciplinary team in the area. A team is therefore already prepared to receive the patient on site, at the emergency room, thus reducing wait times.
The clinics approached as part of the transfer of oncohematology patients were very cooperative. Some 70 patients have already officially been transferred to the network, using a structured approach. A pocket folder containing the patient’s clinical information, as well as follow-up and support tools, is included with the transfer, which is much appreciated by network partners.
The project has also been presented to various audiences in the form of posters and oral presentations. It is now being extended to other regions.
The CCAR is therefore still working with partners from the network and CHU Sainte-Justine, participating in and supporting projects with a component involving both the network and the establishment.
Through its actions, the CCAR is instilling the importance of the continuum of care for patients in all its partners.
